Comparative Results with Two Different Anatomically Pre-contoured Locking Compression Plate for Operative Treatment of Distal Fibular Fracture

Abstract
|
|
|
Objective: The purpose of this study was to evaluate results of distal fibular fractures treated with two different anatomically pre-contoured locking compression plate.
Methods and Materials: Between October 2011 and December 2014, 118 cases of 118 patients were treated by anatomical locking compression plate. Based on the different implant, patients were divided into two groups: Group A: 53 cases of 53 patients; Group B: 68 cases of 68 patients. Mean follow-up periods were 13.3 months (range, 12-36 months). Radiographic outcomes were assessed along with reduction state, bone union period, intraoperative bending of plate. Clinical outcomes were assessed along with the criteria of Meyer and postoperative complications.
Results: Anatomical reduction was achieved in all cases. There was no significant difference in bone union period between two groups (p>0.05). Overall rate of intraoperative plate bending was 55.1% and significantly higher in group A (75%) than group B (38.5%) (p<0.01). The clinical results were excellent or good in both groups (p=0.277). Except one case of proximal locking screw breakage during hardware removal in group A, there was no other significant complications.
Conclusion: While the group B may look better than the A group in Overall rate of intraoperative plate bending, Pre-contoured anatomical locking compression plate showed favorable radiologic and clinical outcomes. However, high intraoperative bending rate due to anatomical mismatch, further studies are needed, focusing on complete fitting of plate to lateral cortical margin of distal fibula.
